Введение. Для пенитенциарной медицины на протяжении нескольких лет сохраняется актуальность проблемы высокой распространенности ряда социально значимых инфекционных заболеваний. Цель работы — анализ организации межведомственного взаимодействия пенитенциарного и гражданского здравоохранения Свердловской области по противодействию распространения социально значимых инфекций на примере туберкулеза и ВИЧ-инфекции. Материалы и методы. Проанализированы нормативные правовые документы федерального и регионального уровней, а также отчетные формы по организации оказания медицинской помощи в учреждениях Федеральной службы исполнения наказаний в условиях эпидемической ситуации по туберкулезу и ВИЧ-инфекции за 2021–2025 гг. Результаты. В пенитенциарных учреждениях Свердловской области заболеваемость туберкулезом превышает аналогичный показатель среди населения в 19,6 раза, заболеваемость ВИЧ-инфекцией — в 13,8 раза. Межведомственное взаимодействие между пенитенциарным и гражданским здравоохранением в Свердловской области предусматривает информационный обмен о поступающих и освобождающихся больных социально значимыми заболеваниями, углубленное обследование пациентов с патологическими образованиями в легких на базе медицинских организаций гражданского здравоохранения, перераспределение нагрузки по обследованию на ВИЧ и назначению антиретровирусной терапии на Свердловский областной центр профилактики и борьбы со СПИД. Заключение. Повышенный уровень заболеваемости социально значимыми инфекциями в пенитенциарных учреждениях является обоснованием организации межведомственного взаимодействия пенитенциарного и гражданского здравоохранения, что обеспечивает преемственность в лечении и диспансерном наблюдении пациентов при перемещении их из гражданской сети в места лишения свободы и обратно, позволяет своевременно выявлять и начинать лечение больных туберкулезом и ВИЧ-инфекцией и достигать снижения заболеваемости туберкулезом среди лиц, содержащихся в исправительных учреждениях, повышения охвата антиретровирусной терапией ВИЧ-инфицированных.
Introduction. Over the course of several years the problem of the high prevalence of a number of socially significant infectious diseases has remained relevant for penitentiary medicine. The purpose of the work is to analyze the organization of interdepartmental cooperation between the penitentiary and civil healthcare of the Sverdlovsk Region to counteract the spread of socially significant infections using the example of tuberculosis and HIV infection. Materials and methods. Regulatory legal documents at the federal and regional levels and reporting forms on the organization of medical care in institutions of the Federal Penitentiary Service of Russia in the context of the epidemic situation of tuberculosis and HIV infection for 2021–2025 have been analyzed. Results. In the penitentiary institutions of the Sverdlovsk Region the incidence of tuberculosis exceeds the same indicator among the population by 19.6 times, and the incidence of HIV infection by 13.8 times. Interdepartmental cooperation between penitentiary and civil healthcare provides for information exchange on incoming and outgoing patients with socially significant diseases, redistribution of HIV testing and prescription of antiretroviral therapy at the Sverdlovsk Regional Center for AIDS Prevention and Control, in-depth examination on the medical organizations of civil healthcare of prisoners with pathological formations in the lungs. Conclusions. The increased incidence rate of socially significant infections in penitentiary institutions is the justification for the interdepartmental cooperation between penitentiary and civilian healthcare, which ensures continuity in the treatment and dispensary supervision of patients when they are moved from the civilian network to places of detention and back, allows timely identification and treatment of tuberculosis and HIV among prisoners, to achieve a reduction in the incidence of tuberculosis among persons in correctional institutions, increase the coverage of antiretroviral therapy for HIV-infected people.
