The Place of Antidepressants and Anxiolytics in the Treatment of Patients with Arterial Hypertension in Modern Living Conditions

Abstract

Introduction. Recently cardiac diseases depression, and anxiety have shown high levels of comorbidity. Over the past 5 years, the COVID-19 pandemic has also made an important contribution to the development of anxiety and depression, which has had a significant impact on the prognosis and quality of life of patients.

The aim of the study is to evaluate the indicators of anxiety and depression in patients with hypertension who have had pneumonia caused by COVID-19 who do not reach the target blood pressure values, as well as to study the effect of psychopharmacotherapy in this category of patients during 3 months of follow-up.

Methods. A prospective study of patients with COVID-19 who had uncontrolled hypertension and increased anxiety/depression was conducted. Patients from 2 groups were compared: group 1 — patients who took only antihypertensive therapy (n = 23), group 2 — patients who used psychopharmacotherapy in addition to antihypertensive therapy (n = 21).

Results. Reactive anxiety was registered in all patients from 2 groups. After 3 months, there was a statistically significant decrease in the high level of reactive anxiety in patients using psychopharmacotherapy. There were no significant changes in the results of reactive anxiety in patients from the comparison group. Depression was detected less frequently than anxiety disorders: group 1 — 3/23 (13,04 %) people (only mild); group 2 — 6/21 (28,57 %) patients (mild — 4/21 (19,05 %); severe — 2 (9,52 %)), p = 0,203. Antidepressants were prescribed only 2 patients with a pronounced level of depression at the initial examination. By the 3rd month of using these drugs, the patients’ depression levels had decreased to moderate. By the 3rd month of follow-up, 8/23 (34,78%) patients in group 1 and 18/21 (85,71 %) patients in group 2 reached the target BP figures, p < 0,001.

Conclusion. Patients with increased anxiety and depression who have uncontrolled hypertension should add psychopharmacotherapy to the main antihypertensive therapy in order to stabilize blood pressure and reduce the level of psychoemotional stress.
